It explains how you can apply for Medicoinds coverage.
Here are the key points:You must have a prescription from a physician, which is usually written by a physician.
You must be a resident of New Jersey who has at least one parent, spouse or child over the age of 21.
You can have one dependent child or you can have two children, each with a different parent, dependent spouse or dependent child.
You have to have lived in New York City or New Jersey for the last three years, or have a New York address for at least six months prior to your prescription renewal.
If you don’t have a NY address, you can still apply for coverage through a third party.
The process may take up to 72 hours to complete.
You must have no medical condition that would be considered a chronic condition under the New York State Medi-Cal plan.
You have to provide proof of income, a proof of dependents, a letter from your doctor that lists your primary care physician and a letter that says you are in compliance with New York law.
You also have to be a New Jersey resident and have paid $1,500 for your Medio-Insurance application.
Your Medicoincs coverage will begin on January 1.
